The EPIC Hearing Service Plan is described in four levels of service, technology and products, and costs. The Hearing Service Plan is offered in four distinct program formats:

• fully insured four tiered hearing care benefit plan;
• a combined hearing & vision benefit plan;
• an ASO savings plan;
• and a discount savings plan.

All four program formats provide access to the four levels of hearing technology/services depending upon the degree of hearing loss.

• Insurance coverages and benefits are applied to all product & service pricing.

5. Billing and collection directly for all professional services and devices (no office co-pays or balance billing by provider)
